"My settings are to save in the Downloads>Screenshots folder but it's only saving in the Downloads folder?"
If screenshots are not saved to the right folder, there may be an extension that conflicts with ours. You can create a new browser profile and only install the Awesome Screenshot extension.

If the download location set in chrome://settings/downloads is D:\Test, and in our Options you specified the subfolder Screenshots, the actual location where the screenshot was saved would be D:\Test\Screenshots.

Though the subfolder you set in our Options shows as
